Fried calamari was some of the best I've ever tasted.
Wood fired margarita pizza was also top notch!
Great selection of cold beers and local-ish wines available. I'll deff return to try a pasta dish and a desert.

WOW!!!! Absolutely the MOST DELICIOUS AUTHENTIC amazing food in all of my travels this far!! Between the house made Mozerella cheese, fresh homemade pizza crust with fabulous flavor, Pizza sauce is perfect, homemade soup was scrumptious I can’t decide what I loved most! Even the salad was out of this world! Service was also top notch! Fast friendly attentive service! And the atmosphere is cozy & nice. Love this place!!! Bravo!

I was in town from Houston for business, and one of my local coworkers recommended this place to try for dinner one night while I was here. Well, my last night in Vernal was that night. I had the Primavera pizza with white sauce, spinach, prosciutto, and added sausage (held the tomatoes). I haven't say that was absolutely fantastic, and I ate the entire pizza. The waitress was friendly, polite, and attentive, which always adds to the dining experience but isn't always found at every restaurant. It is at this one. All the employees seemed to be enjoying their jobs. If you're looking for a great pizza and a nice dining experience, this is the place to come!

Great dinner in basin. Their bread sticks are great, ask for some dipping sauce.

The Fried Calamar appetizer was perfect..Clam dish was SPOT ON !! The shrimp, mussels, calamari dish was a bit of a disappointment.
The calamari was OVER COOKED.
Being a chef, I was a bit let down, but I'm sure it was just an off day .
I WANT TO GO BACK & taste some of the pizza.

Absolutely delicious. One of the best pizza crusts I have ever eaten. It was very tender and light with the perfect amount of rustic flavor from the wood oven. The cheese bread they brought out for an appetizer was absolutely heavenly. The waiter was very knowledgable about the menu and he provided great recommendations and a great experience. The special was not only delicious but a great value as well. We will definitely be coming back soon.

My wife and I had the best experience at Antica Forma last week! The food was amazing. The pizza was delicious and fresh, the Alfredo was creamy and delicious, the caprese was fantastic, and the peach pie was the best we’ve ever had! Everybody needs to go to Antica Forma!

The pizza here was not bad it could use just a little more sauce it felt like a lot of crust. Some of the slices were a little overcooked. The Nutella pizza dessert was great. I highly recommend it. The staff was friendly and helpful.

Went here for the first time for lunch with my husband and had heard many good things. We were seated right away as it wasn’t very busy around 12:30 on a Friday. The food was prepared very quickly and was delicious! We asked the waitress what she recommended and we tried the pistachio pizza and the brisket sandwich and we were not disappointed! We will definitely be going again!
